  • “Everything Looks Normal.” That’s the Most Dangerous Sentence in Medicine.

    You had been tired for two years.

    Not the kind of tired that sleep fixes. The kind that settles into your bones.

    The weight crept up. Your hair thinned. Some mornings the brain fog was so thick you read the same email three times before it finally made sense. You knew something wasn’t right, but every time you brought it up, the conversation seemed to end in the same place.

    So you finally pushed for the lab work.

    You sat across from your provider, waiting for the answer that would explain everything.

    Then came five words.

    “Everything looks normal.”

    You drove home and started doing the math on yourself. Maybe you were lazy. Maybe it was stress. Maybe this was simply what getting older felt like.

    Here’s what should make you angry.

    You were not imagining your symptoms. Your labs were not wrong because you somehow convinced yourself something was wrong. More often than not, the problem is that the results were interpreted against the wrong standard.

    Normal Is Not the Same as Healthy

    One of the biggest misconceptions in medicine is that “normal” automatically means healthy.

    It doesn’t.

    A laboratory reference range is a statistical tool. It tells us where most people fall. It does not tell us how you should feel.

    Lab reference ranges are created using data from the people who show up to have blood work performed. That population is not necessarily healthy. In many cases, it includes people who are already dealing with chronic health concerns, metabolic dysfunction, inflammation, or hormone imbalances.

    When your result lands somewhere within that range, it means your number is common. It does not automatically mean your body is functioning optimally.

    Common and healthy are not always the same thing.

    When “Normal” Misses the Bigger Picture

    This is the gap many people live in for years.

    Their symptoms are real.

    Their labs are technically normal.

    And nobody connects the dots.

    Take thyroid function, for example. A TSH level may technically fall within the laboratory reference range, yet a patient may still experience fatigue, hair loss, constipation, cold intolerance, and difficulty losing weight. The lab says one thing. The body says another.

    Iron is another common example. I’ve seen women told their ferritin level was normal simply because it wasn’t low enough to trigger a laboratory flag. Meanwhile, their hair continued thinning, their energy remained low, and restless legs disrupted their sleep.

    Then there’s blood sugar.

    Most people know about glucose. Fewer people know about insulin.

    A fasting glucose level can remain normal for years while insulin levels quietly climb in the background. By the time glucose finally becomes abnormal, the metabolic dysfunction may have been developing for a very long time.

    The lab wasn’t necessarily wrong.

    It just wasn’t telling the entire story.

    Borderline Is Not a Green Light

    Borderline is not a green light.

    Borderline is information.

    It’s your body’s way of signaling that something may be changing.

    A number that creeps toward the edge of normal year after year tells a story. Unfortunately, many providers only look at the snapshot sitting in front of them. They never look at the trend.

    A laboratory result that gradually worsens over time may be more important than a single abnormal value. Trends often reveal what one isolated test cannot.

    The direction matters.

    Not just the destination.

    How I Read Labs Differently

    When I review lab work, I’m not looking for a single number to explain everything.

    I’m looking for patterns.

    I want to know what your symptoms are doing, what your numbers are doing, and whether those two stories line up.

    That means looking at the entire panel rather than one isolated result. It means comparing your symptoms to your laboratory findings instead of dismissing one to protect the other. It means evaluating where your numbers fall within the range and considering whether they support the way you feel.

    Most importantly, it means looking for root causes instead of simply waiting for a problem to become severe enough to trigger a red flag.

    What You Can Do Right Now

    If you’ve been told that everything looks normal but you still don’t feel like yourself, don’t stop asking questions.

    Start by requesting your actual laboratory values. Don’t settle for a verbal summary. Ask for the numbers and the reference ranges in writing.

    You have a legal right to access your laboratory results, including the exact values and reference ranges. Those results belong to you.

    Next, start tracking your symptoms. Write them down. Include dates, changes, and anything that seems to make them better or worse. Patterns often reveal information that a single appointment cannot.

    Finally, ask a better question.

    Instead of asking, “Am I normal?”

    Ask:

    “What do these numbers mean in the context of how I feel?”

    That question can completely change the conversation.

  • She Was Too Lean to Have PCOS. So Everyone Missed It.

    She was 29 when she landed in my office. Slim. Athletic, even. The kind of body the internet swears cannot have PCOS.

    For nearly a decade, she had been dealing with irregular periods, acne that never responded to creams, hair growing where she didn’t want it, and thinning where she did. Her mood seemed to shift without warning, and no one could tell her why.

    Every provider gave her some version of the same answer. Her weight was fine. It was probably stress. One provider even told her she couldn’t have PCOS because she wasn’t overweight.

    She believed him for six years.

    The problem was that he was wrong.

    The Problem With the PCOS Stereotype

    Somewhere along the way, PCOS became associated with a single image: an overweight woman struggling to lose weight.

    While weight gain is a very real symptom for many women, it is not a requirement for diagnosis. Unfortunately, that stereotype has caused countless women to be overlooked, dismissed, or told that their symptoms must be caused by something else. The result is years of frustration, confusion, and unnecessary suffering.

    The textbook image of PCOS is often an overweight woman. But lean women can absolutely have PCOS. Thin does not automatically mean healthy, and it certainly does not rule out a hormone or metabolic disorder.

    I’ve seen women in a size 4 struggle with PCOS. I’ve seen women in a size 24 struggle with PCOS. Weight may be part of the story, but it is rarely the entire story. When we reduce PCOS to a number on the scale, we often miss what may actually be happening beneath the surface.

    The Name Has Been Confusing Women for Years

    Here’s something I’ve never fully bought: the idea that PCOS lives or dies on whether cysts appear on your ovaries.

    It never sat right with me.

    I’ve seen women with every classic sign of PCOS who had completely normal ultrasounds. I’ve also seen women with harmless ovarian follicles who were quickly labeled and sent home with birth control without anyone taking the time to understand what was actually happening. The name has always pointed many people toward the wrong organ.

    I was trained to look at the whole person. Actually, I was taught a much simpler rule.

    If it walks like a duck and talks like a duck, it’s probably a duck.

    Irregular cycles, elevated androgens, acne, hair changes, and insulin resistance. That’s the duck.

    The ovaries are just one feather.

    What PCOS Is Actually About

    For many women, insulin resistance sits at the center of the picture. The body may continue producing more and more insulin even when blood sugar levels appear normal. Over time, elevated insulin can disrupt ovulation, influence hormone production, worsen acne, increase androgen levels, and make weight management significantly more difficult.

    This is why two women with PCOS can look completely different while sharing the same diagnosis. One woman may struggle with infertility. Another may struggle with fatigue and weight gain. Another may battle acne and hair loss while maintaining a healthy body weight.

    The symptoms may look different, but the underlying story is often much more complex than a number on a scale.

    How I Actually Evaluate PCOS

    I don’t start with the ultrasound.

    I start with the story.

    Your cycle history, your symptoms, when things changed, and the patterns you’ve noticed over time. The pattern often tells me more than a single laboratory value ever could.

    From there, I look at the hormones and metabolic markers that are frequently overlooked. Depending on the individual, that may include total and free testosterone, DHEA-S, fasting insulin, Hemoglobin A1c, thyroid testing, prolactin, SHBG, and additional hormone testing when appropriate.

    For some women, specialty testing may provide an even deeper look into the hormonal and metabolic picture.

    An estimated 70% of women with PCOS may not know they have it. Not because the condition is rare, but because it is frequently overlooked, misdiagnosed, or never fully investigated.

    PCOS Is Now PMOS

    In 2026, after years of international discussion and research, experts officially announced a new name for the condition: PMOS, or Polyendocrine Metabolic Ovarian Syndrome.

    The new name acknowledges something many clinicians have recognized for years. This condition is not simply about ovarian cysts. It involves hormones, metabolism, and multiple systems throughout the body. The new name better reflects what women have been experiencing all along.

    In many ways, the science finally caught up with the pattern.

    The name change followed a 14-year global effort to better reflect the hormonal and metabolic nature of the condition. The goal was to move beyond the misconception that ovarian cysts are the defining feature of the disorder.

    What This Means for You

    If someone dismissed your symptoms because your weight looked fine, that wasn’t a complete evaluation.

    If someone dismissed your symptoms because your ultrasound looked normal, that wasn’t a complete evaluation either.

    Your body deserves more than assumptions.

    Track your symptoms. Track your cycles. Ask questions. Seek answers. Most importantly, don’t let your clothing size determine whether your concerns are taken seriously.

    Women of every size can develop PCOS.

    Women of every size deserve answers.
